To submit a death claim form, you can download and print the relevant form using the link below to send to us. Full instructions including a checklist are also provided to help you.
Scottish Friendly Life Policies
If the person who has died has a Life policy with Scottish Friendly, you can notify us and make a claim by completing a death claim form.
Scottish Friendly Pension Policies
If the person who has dies has a Pension policy with Scottish Friendly, you can notify us and make a claim by completing a Notification of Bereavement form (pensions).

What you need to send us
In addition to the above death claim form, we will require a valid copy of the death certificate and the original policy documents if available.
We will also let you know if you need to send us a nomination form, grant of confirmation/probate and a deed of assignment.
